Home Services For Individuals Supports Coordination Services
Goodwill’s Supports Coordination Services are for people with intellectual and developmental disabilities. These services take place in their community.
Each person establishes a Person-Centered Plan of Service (PCPS). This plan develops social networks and encourages support from family and the community.
This plan aims to:
Identify the person’s needs and goals.
Match the person’s needs with the correct services.
Schedule all appointments and meetings.
Address any concerns about the person’s work and housing situation.
For additional employment and training assistance, other Goodwill programs are also available. This includes Skill Building, supported employment services and other job training programs. Goodwill of Greater Detroit is a barrier-free facility.
Program Requirements:
- Person must have an intellectual or developmental disability.
- Person must be 18 years old or older.
- Person must be a resident of Detroit or Wayne County.
As Goodwill’s Prepaid Inpatient Health Plan (PIHP) Detroit Wayne Integrated Health Network (DWIHN) funds all Goodwill Behavioral Health programming including Supports Coordination and Skill Building services.
To receive Behavioral Health Services for Supports Coordination or Skill Building an individual must be at least 18 years old with a primary axis one diagnosis of Intellectual or Developmental Disability (I/DD).
